Membership Terms

This membership agreement between Vital Skin & Body LLC. and ______________________________ (Member name) shall be effective on the date ____/____/_____. Benefits will become available after the above billing date and can be prorated to start the program based on $1.00 per day.

Memberships include $1.00 off per unit of any neurotoxin we stock (Botox, Jeuveau, Daxiify and Xeomin). Brands may change. A minimum of 20u per appointment is required.

Membership is non-transferable or shareable.

Manufacturer coupons can be applied, but Promotions with Vital Skin & Body cannot be further discButtonounted but can be purchased for additional treatments.
